Transition of care or graduating from pediatric dentistry is a big milestone. For many, the process can be daunting. Pediatric and general dentists have essential roles in this transition. This presentation will discuss the concept of health care transitions as well as the transition pathway, process and discharge of adult patients seen at a tertiary children's hospital in South Australia.
