Introduction: the space between canines and primary molars must be kept intact, as it determines the preservation of the space necessary for the eruption of canines and premolars. Ectopic eruption can affect that space, due to an abnormal mesioangular eruption of the first permanent molar, which leads to an impaction and resorption at the distal surface of the second primary molars. The objective of this case report is to present an conservative management of severe ectopic eruption.
Case report: male patient, 7 years 4 months, Frankl 2, first stage mixed dentition, high cariogenic risk in caries activity. Supernumerary in relation to tooth 6.3. Tooth 5.5, without mobility, with penetrating mesial ICDAS 5 and root resorption, associated with posterior attack of the Korkhaus supporting zone. Due to pain in tooth 5.5, trepanation was performed, leaving gelite and glass ionomer. In future sessions and due to discomfort in relation to the area, interproximal enamel reduction of tooth 5.5 was performed, progressively, in order to eliminate the contact point between both teeth, using a high speed dental handpiece, and to avoid damage to the surface of tooth 1.6, glass ionomer was used. Due to the risk of fracture of the affected tooth and poor cooperation of the patient, a conservative treatment was carried out. Once the cooperation of the patient has improved, other treatments would be considered.
