Introduction: Drug induced gingival hyperplasia is an overgrowth of the gingiva in a medically compromised patient. The gingival overgrowth can be so excessive that it compromises oral hygiene, mastication, can lead to disfigurement and even compromise the airway. Drug induced gingival hyperplasia can be a side effect of anticonvulsants, immunosuppressants and calcium channel blockers.
Case Report: This presentation discusses an 11 year old male patient of record at Children's Wisconsin who was consulted by dental for gingival hyperplasia in July of 2022. The patient had gingival hyperplasia localized to the anterior mandibular gingiva. The patient’s medical history was significant for hypertensive cardiomyopathy with chronic heart failure from systolic dysfunction and left ventricular dilation secondary to hypertension in the setting of chronic oliguric renal failure. He received a kidney transplant in 2020. The patient was previously on amlodipine, a calcium channel blocker, to control blood pressure and his physician halted the medication after June 2022. The patient was seen again in September 2022, and the gingival hyperplasia had resolved. On February 17, 2023 the patient presented with granulomatous like gingival overgrowth in the maxillary left quadrant. The p</span>atient was re-evaluated on March 14, 2023 and presented with increased expansion of gingival overgrowth in maxillary left quadrant and gingival swelling had spread to all areas of the mouth. A full mouth gingivectomy and biopsy was performed under general anesthesia. The specimens were sent for histological examination. The report will include clinical and histological findings and treatment completed for the patient.
