Improving Dental Health Outcomes in Pediatric Kidney Transplant Recipients

Purpose: The objective of this ongoing quality improvement project is to improve oral health for pediatric kidney transplant recipients at our institution by standardizing the pre-transplant dental clearance process, implementing an educational intervention, and improving compliance with basic dental care pre- and post-transplant. This project’s aims are to increase the percentage of patients with a dental home and increase patient understanding of the importance of oral health in overall transplant care.
Methods: A key driver diagram was outlined after discussion with stakeholders. Baseline data was obtained through a survey assessing pediatric kidney transplant patients' demographics, dental history, and perceived importance of oral health. An educational intervention was carried out following completion of the survey. A pediatric dental resident delivered education, provided oral screenings, and facilitated dental visits. Topics discussed include home care and the relationship between systemic and oral health. Outcome measures included frequency of dental visits, dietary habits, oral hygiene, and perception of the importance of dental health. A follow-up survey was administered to track changes.
Results: Nineteen patients completed the initial survey, received the educational intervention, and completed the follow up survey. Over half of the patients benefited from assistance from the dental resident in facilitating dental care. Survey results showed an improvement in the perceived importance of oral health following the educational intervention.
Conclusion: Findings suggest the need for improved dental education and care for pediatric kidney transplant recipients. The addition of a pediatric dentist to the kidney transplant team is warranted.
