386 - Evaluation of Regenerative Endodontic Methods for Immature Necrotic Permanent Teeth

Purpose: Achieving a blood clot is a necessary step of regenerative endodontic treatment. Factors associated with the formation of blood clot/success of regenerative endodontic procedures were explored for immature, necrotic permanent teeth.

Methods: This retrospective cohort study examined patients with immature, necrotic permanent teeth treated with regenerative endodontic treatment in Boston Children’s Hospital Dental Clinic. Primary outcome was whether or not a blood clot was able to be achieved during regenerative endodontic treatment. The secondary outcome was to assess the various factors (age, tooth positioning, trauma, caries, swelling, sinus tract, pain, radiographic pathology, antibiotic intake, number of visits, Moorrees stage of root development, irrigation concentration, number of calcium hydroxide (CaOH) applications), that contribute to the formation of a blood clot in regenerative endodontic treatment. Chi-square tests were completed to measure associations between the primary and secondary outcomes.

Results: Thirty-two patients (mean age 8.6 years) were included in the study. More teeth (14/18, 82%) that received a single application of CaOH achieved a blood clot (P <.05). There is also a statistically significant association between the presence of blood clots and the number of visits, indicating that these variables are not independent of each other. No significant associations were found between the other factors and success achieving a blood clot. Logistic regression model found that there is a positive association between number of CaOH applications and the achievement of a blood clot (Odds Ratio: 4.666667, 95%CI: 1.005802- 21.65215).

Conclusion: Number of visits and applications of CaOH was significantly associated with success of achieving a blood clot for regenerative endodontic procedures, which may indicate that less complicated cases have a greater chance of success with regenerative endodontic treatment.
