[Introduction]
A nine-year-old boy with upper left central incisor impaction was treated by ISW (Improved Super-elastic Ti-Ni alloy wire, developed by Tokyo Medical and Dental University) will be discussed.
[Case Report]
A nine-year-old boy with a chief complaint of upper left central incisor delay eruption came for orthodontic evaluation with his family. Clinical examination revealed skeletal Class I relationship and convex profile appearance. Panoramic film and CBCT image showed upper left central incisor horizontal impaction. We arranged surgical exposure under general anesthesia. Orthodontic traction was started one week after the surgery. Due to the characteristics of super-elasticity, ISW can be engage into impaction tooth in the early treatment. With ligature wire and elastic chain traction, the active treatment was finished in 9 months. The impaction tooth was tractioned to proper position. Fixed retainer was boned to four anterior teeth. In central incisor impaction case, it might be difficult to decide when and how to traction. We will discuss the proper time and surgery protocol of central incisor impaction traction. Impaction tooth is encouraged to be tractioned when the root is at least two-thirds of its eventual root. In this case, early traction with ISW simplified the treatment, after 9 months, a desirable result was achieved, and the patient was pleased with the treatment outcome.
